One more possibility sits outside your control. Instagram notes that in certain regions it may disable accounts for people in a specific age range to comply with local law. If your notice mentions age, follow what it says; an appeal about content will not help.
Which form matches your disable reason
For most reasons there is no separate form to find. The review lives on the screens Instagram shows after you log in, and the old links passed around forums mostly lead back there. What changes with the reason is what you should prepare.

Instagram does host one standalone appeal form. Its description limits it to accounts disabled for repeat intellectual property violations or for sharing fraudulent or misleading content, and says other cases may not receive a response, so for most people it is not the route. ID verification: what Instagram accepts
Be ready to prove who you are. Instagram may ask for a document, a selfie that matches it, or a video selfie.

Three rules from Instagram's ID page decide whether the check passes first time:
- Send only what is asked for. Instagram says submitting more documents than necessary may delay the review.
- Do not edit the image. Photos digitally modified to hide information are not accepted. Cover non-essential details, such as an ID number, with paper before you take the photo.
- Use a secure email address. The result of a video selfie review is emailed to the address you provide. A pass may come with a link to reset your password; after a fail you may be able to resubmit.
While a video selfie is under review, Instagram says you will not have access to the account. Declining the selfie is allowed, but Instagram warns that you may then be unable to log back in.
How to write the appeal
Some review screens are a single button and some give you a text box. If you get the box, four short parts are enough: who you are, the reason you were given, why you believe it is wrong, and what you are asking for.
Username: @[handle]. Email on the account: [email]. Disabled on: [date]. The notice says the account was disabled for [exact wording from your screen]. I believe this is a mistake because [one or two specific facts tied to that reason: what the account posts, what the flagged content was, or that the activity was not mine]. Please review the decision. I can provide ID or any further information you need. [Full name, as it appears on your ID]
- A long story about what the account means to you
- "I did nothing wrong", with no reference to the reason shown
- Anger, threats or legal language
- Claiming you were hacked when you were not
- The same message sent again every day
- Under about 120 words
- Quotes the reason from the notice
- One or two facts that answer that reason
- Says so plainly if the activity was not yours
- Sent once, from your usual phone
If someone else did have access, say so and say what you have secured since. It is relevant, and it is the kind of thing a reviewer can check. Do not invent it.

The nearest published figure is for content, not accounts: Instagram says a review of a removed post can take up to 90 days. Use that to set expectations. Weeks of silence are normal and do not mean the appeal failed.
While you wait: what helps and what hurts
- Do not open a replacement account. Meta says it works to disable accounts created to get around its restrictions.
- Do not pay a recovery service. Instagram says Meta has no general public phone number for support and that numbers claiming to be one are potentially scams. - Check email daily, spam included. Requests for more information go there, and answering quickly is the one thing you control.
- If you are in the EU, Instagram's support page links a separate form that routes your issue and, in some cases, emails you a reporting number.
- If the account had Meta Verified, its support covers "general access or security issues" for the subscribed profile. The support page does not say agents can reverse an enforcement decision.
When you get it back
Change the password, turn on two-factor authentication, revoke every third-party app you do not use and read Account Status to see what is still on the record. Meta says a successful review removes the strike along with the restriction, but anything you did not appeal stays.
Then remove the cause.
